Gathering Our Voices 2012
Welcome to Registration for Gathering Out Voices 2012:
This year we have introduced an online registration procedure.
To register for Gathering Our Voices follow these five simple steps:
Step 1: Before you begin registration please ensure that you gather the following information for all participants (both chaperones and youth):
- First & last names
- Birthdates
- Gender
- Emergency contact information (name of contact & phone number)
- Medical Number
- Any medical or food allergies
As Primary Contact you will also need to have the following information:
- Affiliation (i.e. Group name, First Nations Band, Organization, etc.)
- Full Mailing Address
- Contact information including Phone, Fax & Email
- Method of payment
Step 2: Download the electronic registration form: GOV 2012 Registration Form
Step 3: Save it to your desktop and include the affiliation of your group. For Example:
“Victoria Native Friendship Centre Conference Registration”, or
“Tsawout First Nation Conference Registration”
Step 4: Fill out the registration form. If incomplete, a prompt at the bottom will inform you on what is missing. Mandatory items are marked with an asterisk (*).

You have now finished the electronic portion of the GOV 2012 registration. The Youth Conference Administration Assistant will contact you to confirm payment details.
To Print: If you would like to print the completed form for your files, first select the rows and columns you would like to print. Then in your print menu choose ‘Print Selection’ NOT ‘Print Active Sheets.’
*** Do not print the entire worksheet or a very large print job will be generated. ***
